ACTORS & STAGE MANAGERS

Acting & stage management company members should submit the following:

• Two signed copies of your contract
• W4 form and AR4EC form (for payroll tax withholding)
• Completed direct deposit enrollment (optional)
• I9 form and, attached, a copy of either (1) your passport OR (2) your driver's license AND social security card.

DESIGNERS & DESIGN ASSISTANTS

All artists who work on a contracted basis should submit the following:

• Two signed copies of your contract
• W9 form
